Notice of Privacy Practices
This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our duties
Los Quiropracticos Colorado is required by law to maintain the privacy of your protected health information (PHI), provide you with this Notice describing our legal duties and privacy practices, follow the terms of the Notice currently in effect, and notify you if a breach of unsecured PHI occurs.
How we use and disclose your health information
Treatment: we use your PHI to provide chiropractic and physical therapy care and to coordinate care with other providers.
Payment: we use and disclose PHI to bill auto insurance, workers' compensation carriers, health insurance, and attorneys on your behalf.
Health care operations: we use PHI for quality review, training, scheduling, and business management.
We may also disclose PHI as required by law, for public health activities, to report suspected abuse or neglect, for health oversight, judicial proceedings, law enforcement, organ donation, research with safeguards, to avert a serious threat to health or safety, and for workers' compensation as authorized by state law.
Any other use or disclosure requires your written authorization, which you may revoke at any time in writing.
Your rights
Access: you may inspect and request a copy of your PHI we maintain.
Amendment: you may request that we amend PHI you believe is incorrect or incomplete.
Accounting of disclosures: you may request a list of certain disclosures we have made of your PHI.
Restrictions: you may request restrictions on certain uses and disclosures. We are not required to agree to every request.
Confidential communications: you may request that we contact you in a specific way or at a specific location.
Paper copy: you may obtain a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.
Breach notification: we will notify you if a breach of your unsecured PHI occurs.
How to file a complaint
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
Changes to this Notice
We reserve the right to change this Notice and to make the revised Notice effective for all PHI we maintain. The current Notice will always be posted in our office and on this website.
